Dear Parents/Guardians,
Welcome to Lakewood Elementary School. We are happy to have you at our school, and we would like to make the enrollment process as easy as possible. Please read the following information to ensure that you have proper information and enrollment documents.
A student must be enrolled by one of the following:
A Parent with legal custody.
A Guardian, who has proper documentation, notarized statement given guardianship.
Enrollment Checklist
A student must provide the following:
Withdraw Form and/or last report card from previous school
Original Birth Certificate
Original Social Security Card
Original Shot Record
Parents Identification
Proof of Residency (Utility Bill, Lease, Tax Statements)
