Always keep the office & nurse informed about your child. We need to know about doctor’s appointments, and serious illness.
Please notify us anytime your contact/emergency numbers change. Complete and update the emergency medical form indicating any known food, drug, or “other” type of allergy your child may have.
Medications or treatments given at school must be accompanied by a doctor’s order and in original container from pharmacy with appropriate information on the label.
PRN (as needed) orders are to be given at home in most cases- excluding MDI’s (Meter Dose Inhalers) containing meds for Asthma, for example- which still requires a doctor’s order and a permission form completed and signed by the parents.
Teachers must send the student to the nurse with a written pass which includes the name of the student and teacher as well as contact name and telephone number if possible.
A student that is going home due to illness or injury can only be signed out by the contact persons listed on file. Parents, please check in at the front office and remember to bring proper ID when signing your child out of school.
A student that is returning to school from absence due to illness or injury should bring a written statement from the parents or a doctor stating the reason for the absence, the condition of the student.
Immunizations need to be current with a (copy) of your child’s shot record on file.
All medicine is to be brought to and kept in the school nurse’s office.
All prescription medication will only be dispensed by the school nurse and/or other stuff members, and will not be carried by the student. All prescription medication must be accompanied by a medication form signed by a parent or guardian giving authorized school personnel directions for its administration (time & dosage), along with physician signature.
Over-the-counter (OTC) medication will only be dispensed by the school nurse and/or other staff members and will not be carried by the student. All OTC medications must be brought to the school by a parent in the original and unopened container with a signed permission release. The permission release must be renewed every two weeks.
School personnel will not give any medicine, including Tylenol, unless it is provided by the parent or guardian in the appropriate manner as stated above.
Students needing occasional medication, such as penicillin, etc., for colds, earaches and sore throats, are to take those medication at home if possible. Medication that is prescribed three (3) times a day can be giving before the student comes to school, after school, and again at bedtime.
Each school year an annual renewal medication form must be filled out by your doctor for your child to take medication. Parents are responsible for picking up remaining medication and equipment from school or it will be properly destroyed.
REMINDER: Please remember, according to North Forest ISD medical policy, that a note from the parent and a signed medication form from the doctor must be completed and returned to the school nurse on the first day of school. It is also acceptable to use the doctor’s forms. The medication needs to be in the original labeled bottle. No medication form will be carried over from the previous school year. Medication will not be accepted without these forms.
NOTICE: North Forest ISD has a policy that students are not able to transport medications of any kind to and from school. All medication (prescription and non-prescription) must be delivered and picked up by a parent, guardian, or an authorized adult.
These restrictions are necessary for protection of the health and safety of your child. We will appreciate your cooperation in this manner.
